It is uncommon — but it is possible.A root canal treated tooth no longer has a living nerve, so it cannot develop new decay-related infection from within. However, if the seal placed at the end of the root canal fails over time, or if the filling/crown above it allows bacteria to re-enter, reinfection can re-occur inside the tooth and end up at the root-tip. This is a question worth thinking through carefully — and the honest answer is: it depends.As a general principle, a healthy natural tooth — even one that has been root canal treated — is preferable to any artificial replacement. The most common filling material used today is composite resin — a tooth-coloured material that bonds directly to the tooth. It is both functional and natural-looking, and because it bonds to the tooth rather than simply filling the space, it often allows more of the natural tooth structure to be preserved.Amalgam — the older silver filling material — is still occasionally used in certain clinics, though it has become significantly less common. Not necessarily — but old fillings do warrant monitoring.Fillings are not permanent. Over time, they can crack, wear, chip at the margins, or allow a small gap to form between the filling and the tooth — creating a space where bacteria can enter and decay can begin again beneath the restoration. This is called secondary decay, and it is one of the most common reasons patients who have had fillings for years find themselves needing more extensive treatment.An old filling that is intact, well-sealed, and showing no signs of deterioration may not need to be touched. One that is beginning to fail is better addressed early — before the decay beneath it deepens.The goal is never to replace a filling before it needs replacing. It is to catch the ones that are beginning to fail before they become a larger problem that may or may not need root canal treatment.

A filling replaces the missing or decayed portion of a tooth. A crown covers the entire tooth above the gumline — restoring its shape, strength, and function from the outside in.The distinction becomes important when the amount of remaining tooth structure is the question. And the honest answer is that it depends on a set of specific factors that need to be properly assessed before any recommendation is made.The factors we consider include:Position and angulation. A wisdom tooth that is fully erupted, correctly aligned, and accessible for cleaning is in a fundamentally different clinical situation from one that is partially erupted, angled toward the second molar, or deeply embedded in bone. Position determines both the risk the tooth poses and the complexity of removing it.Cleanliness. Even a fully erupted wisdom tooth can become a problem if its position makes it genuinely impossible to clean. The most posterior tooth in the mouth is always the hardest to reach, and a wisdom tooth that accumulates plaque consistently will eventually develop decay — on itself, or on the second molar directly in front of it.History of symptoms. A tooth that has caused repeated episodes of swelling, pain, or infection tells a different story from one that has been entirely silent. Recurrence matters: a single episode may resolve without removal; repeated episodes usually indicate a structural problem that will not resolve on its own.The condition of adjacent teeth. Some wisdom tooth positions result in gradual pressure on, or decay developing against, the second molar. It is informative.A wisdom tooth that hurts periodically — flaring up for a few days, then settling, then flaring again weeks or months later — is almost always describing a specific pattern. The gum around a partially erupted wisdom tooth becomes inflamed; the inflammation eventually resolves with or without treatment; the underlying situation that caused it has not changed; and so the same cycle begins again at the next opportunity.This is called pericoronitis — an infection of the soft tissue surrounding a partially erupted tooth — and it is discussed in detail in the next question.The important thing to understand about intermittent wisdom tooth pain is this: each resolution of symptoms is not a resolution of the problem. The gum has settled. The tooth has not changed. The flap of tissue that traps food and bacteria is still there. The conditions that caused the first episode are identical to the conditions that will cause the next one.The fact that it comes and goes is not evidence that it is manageable long-term. In many cases, it is exactly the pattern that signals the tooth needs to be addressed before the next episode is worse than the last.Whether removal is the right response depends on the full clinical picture. Pericoronitis is an infection of the gum tissue that partially covers a wisdom tooth that has not fully erupted.When a wisdom tooth breaks through the gum only partway, it leaves a flap of gum tissue — called an operculum — overlying part of the crown. This flap creates a space that is warm, moist, and extremely difficult to clean: precisely the conditions bacteria require to multiply. Food debris and plaque accumulate beneath the flap, the tissue becomes inflamed, and infection follows.The severity can range from mild localised soreness to significant swelling extending into the jaw, cheek, and lymph nodes. In more serious episodes it can affect swallowing or the ability to open the mouth fully. In rare cases, untreated pericoronitis can progress to a spreading infection requiring hospital management. Most episodes, when managed promptly, do not reach that point — but the risk is real, and it is one of the reasons pericoronitis should not be left to resolve on its own without assessment.The reason pericoronitis keeps recurring is structural. Irrigation and a course of antibiotics can resolve an acute episode. They cannot change the anatomy of the tooth and the flap. Once the antibiotics are finished and the inflammation settles, the operculum remains. It will collect debris again. The bacteria will return. The infection will follow.This is why pericoronitis that has occurred more than once is generally considered an indication for removal — not because the current episode is dangerous, but because the recurrence is predictable, and the pattern tells us what is coming.In cases where removal is not immediately appropriate — because the patient is acutely infected and needs to be settled first, or for other medical reasons — we may discuss operculectomy: a minor procedure to remove the overlying flap of gum tissue. This can reduce the risk of recurrence in selected cases, though it is not a universal solution and we will explain clearly when it is and is not appropriate.

The experience of wisdom tooth removal varies considerably depending on the position and anatomy of the tooth — which is one of the reasons a proper clinical assessment and X-ray is necessary before anything is planned.Simple extraction A wisdom tooth that has fully erupted and has straightforward roots can often be removed in a manner similar to any other tooth extraction: local anaesthesia, elevation of the tooth from its socket, and removal.
